Thank you for choosing Fort Sanders Neurosurgery and Spine for your healthcare needs. It is always our goal to provide the best possible neurosurgical care.   

Whether you were referred to us by your primary care physician or another specialist, referral to a neurosurgeon is an emotional experience for you and your family. We understand you may have many questions, and we strive to alleviate your concerns by communicating with you throughout the continuum of care you receive at our practice.

Parking

We are located at 501 20th Street in Suite 203, Knoxville, TN 37916. Ample parking is available in the parking garages located near our office in the Fort Sanders Professional Building.

New Patient Registration

On your first visit, you will be asked to fill out registration forms. You may choose to pre-register by downloading, printing and completing the form below. Just bring it to your first visit. Completing the form prior to your visit will help speed up the new patient registration process.

Patient Reminders

We ask that you please arrive 10-15 minutes prior to your scheduled appointment. Please bring the following:

  • Your current health insurance information – to ensure accurate and appropriate billing of your health care services
  • Your current Photo ID
